Don’t Miss Your Chance to Enroll or Change Your Benefits

Open Enrollment is your annual opportunity to make changes to your benefits for the upcoming year. The elections you make during Open Enrollment will remain in effect January 1 – December 31, 20XX unless you experience a qualified change in status (such as marriage, birth or adoption of a child, etc.).

Everyone must enroll for this year’s Open Enrollment. You need to enroll or decline benefit coverage by the deadline, or you and your family will not have benefit coverage beginning January 1, 20XX.

Select Your Year Coverage
Decide if you need to make changes to your current benefits, want to enroll in a new plan, or contribute to an FSA or HSA for 20XX. If you’re adding any dependents to your coverage, make sure you have all the information you’ll need – date of birth, Social Security number, and supporting documentation.

Enroll in Benefits
Set a reminder on your calendar to enroll early – beginning Month Day. Remember, Month Day is the deadline.
